What you send us

Our forms collect your name, email, selected coverage interests, preferred contact method, and any optional phone number, ZIP code, current carrier, referral information, and message you provide, plus optional declarations-page documents you choose to upload. We record when you submit the request and your agreement to be contacted.

How requests are used

Magnolia Insurance Advisors uses this information to review your inquiry and contact you about insurance services. Submitting this form does not authorize marketing text messages or bind, change, or cancel coverage. These forms do not automatically transfer your information to an insurer, CRM, or email attachment. When notifications are configured, Zapier or the email service receives the agency’s notification address and a protected record link, without customer details or uploaded documents.

Storage and access

Submitted form details, document filenames, internal notes, and activity records are encrypted by the application before database storage. Uploaded document contents are encrypted before storage in a private object bucket. Document downloads require authorization and are recorded in activity history. Access to the lead inbox requires sign-in with an authorized agency account. Authorized admins may also review and update records through the site’s connected ChatGPT tools, which use the same access checks. The website uses Sites hosting and Cloudflare infrastructure to process and store requests. Hosting providers may process technical information necessary to operate and protect the service. No internet service can guarantee absolute security.

Information to leave out

Do not put Social Security numbers, payment details, or medical information in your message. You may optionally upload declarations pages using the dedicated upload field. Share only documents you are authorized to provide; redact unrelated sensitive information first. Unsubmitted entries stay in the page’s temporary state; the form does not save them in your browser’s local storage.

Retention and requests

Submitted requests, documents, internal notes, and activity history remain in the agency’s lead inbox until an authorized administrator deletes the record. Hosting backup retention may differ from the active inbox. Technical safeguards

The form limits request sizes, validates fields, and restricts repeated submissions. For abuse prevention it temporarily stores a keyed, non-reversible identifier derived from the connection’s IP address, rather than the raw address, in its rate-limit database. File types and sizes are checked, but uploaded documents are not malware-scanned. These rate-limit records expire after approximately two hours and are cleared during subsequent submissions. Hosting logs may retain separate technical connection information.
